Levels of Care
 
Level I
Activities of Daily Living (ADL)

• Capable of ambulating or transporting self to the dining room
• Can ambulate short distances or can use the walker/wheelchair independently for long distance
• Transfers in/out of the bed and in/out of the chair independently
• Capable of bathing self or may require assistance with back and lower extremities
• Care for self at toilet completely; no incontinence/manages incontinence
• Dresses and undresses and selects clothes from own wardrobe
• Eats without assistance
• Normal-oriented to time, place and person. Able to plan daily events.
• Medications may either be self-administered or administered by the licensed staff

Independent Activities of Daily Living (IADL)
• Able to look up phone numbers, dial numbers and answer the phone
• Has the ability to access St. Leonard’s transportation system
• Shops without help and purchases both large and small items
• Capable of filling out menu, select items from buffet and carry them to their table
• Performs light tasks (i.e. making bed) maintain acceptable level of cleanliness
• Manages financial matters
• Does personal laundry

Level II
Activities of Daily Living (ADL)

• Needs help getting in and out of shower/tub. Can wash face and hands but cannot wash the rest of body
• Dresses and undresses self with minor assistance. Needs assistance in selection of clothes
• Ambulates with assistance of assistive devices
• Grooms adequately. Requires supervision
• Independently manages incontinence. May require assistance 1x/week to manage incontinence
• Eats without assistance. May require food to be cut
• May have some fluctuation in memory but can plan daily events appropriately
• Medications Administered

Independent Activities of Daily Living (IADL)

• May be able to answer phone or dial operator in an emergency
• May be able to shop, but requires assistance and someone to accompany them
• May require assistance in carrying food from the buffet and filling out menu
• May require help with bed making and straightening room
• May be able to launder small items, other items completed by staff or family
• May be able to manage daily buying needs, requires help in managing checkbook and paying bills
• May travel but needs someone to travel with/cannot make transportation arrangements

Level III
Activities of Daily Living (ADL)

• Does not wash self, but is cooperative with those who bathe him/her
• Requires major assistance in dressing but cooperates with efforts of others to help
• May need escort/transportation to and from dining room and activities
• Needs total grooming care, but can remain well groomed after help from others
• Needs assistance getting on and off toilet. Requires assistance to manage incontinence daily
• Feeds self with moderate assistance and is untidy
• Frequent fluctuations in memory and orientation causing inability to plan daily events. Requires frequent cueing. Judgment impaired
• Medication administers

Independent Activities of Daily Living (IADL)
• May be able to answer phone, unable to call out or use phone book
• May be able to go to store, but requires assistance in decision making for items. Family does shopping for resident
• Unable to do laundry
• Unable to manage money
• Unable to travel without escort/cannot make any travel arrangements
