Activities of Daily Living (ADL)
Capable of ambulating or transporting self to the dining room
Can ambulate short distances or can use the walker/wheelchair independently for long distance
Transfers in/out of the bed and in/out of the chair independently
Capable of bathing self or may require assistance with back and lower extremities
Care for self at toilet completely; no incontinence/manages incontinence
Dresses and undresses and selects clothes from own wardrobe
Eats without assistance
Normal-oriented to time, place and person. Able to plan daily events.
Medications may either be self-administered or administered by the licensed staff

Able to look up phone numbers, dial numbers and answer the phone
Has the ability to access St. Leonards transportation system
Shops without help and purchases both large and small items
Capable of filling out menu, select items from buffet and carry them to their table

Needs help getting in and out of shower/tub. Can wash face and hands but cannot wash the rest of body
Dresses and undresses self with minor assistance. Needs assistance in selection of clothes
Ambulates with assistance of assistive devices
Grooms adequately. Requires supervision
Independently manages incontinence. May require assistance 1x/week to manage incontinence
Eats without assistance. May require food to be cut
May have some fluctuation in memory but can plan daily events appropriately

Requires major assistance in dressing but cooperates with efforts of others to help
May need escort/transportation to and from dining room and activities
Needs total grooming care, but can remain well groomed after help from others
Needs assistance getting on and off toilet. Requires assistance to manage incontinence daily
Feeds self with moderate assistance and is untidy
Frequent fluctuations in memory and orientation causing inability to plan daily events. Requires frequent cueing. Judgment impaired
