The Health & Rehabilitation Center at St. Leonard is a 120-bed Skilled and Intermediate care facility. Our staff of trained professionals makes every effort to exceed our resident’s expectations of care over-seeing the safety and well being of each resident twenty-four hours a day. Our Health & Rehabilitation Center is Medicare and Medicaid certified.

Residents of St. Leonard Health & Rehabilitation Center enjoy not only the highest-quality care but also have at their disposal the added conveniences of housekeeping services, wander guard and individual alarm systems, notary services, laundry services, beauty salon and barber shop and cable TV. The center features a chapel and daily mass is available for residents. Our staff is on-site around-the-clock and is sincerely dedicated to preserving the physical, emotional and spiritual well being of all our residents.

Nursing personnel are available to facilitate appropriate medical treatment and personal physicians are encouraged to follow and manage residents’ care while they are in residence.
